NGC does not recommend a treatment protocol until the cause of infertility is evaluated. Every patient undergoes an individualised diagnostic workup — many components of which can be arranged at home before your first visit.

    Your existing test results, previous IVF reports, imaging and surgical history are reviewed by an NGC fertility specialist remotely before any additional testing is recommended.

  • Online Consultation with a Fertility Specialist

    A video consultation allows the doctor to assess your situation, answer your questions, and outline the diagnostic or treatment pathway. This can be completed before traveling to St. Petersburg.

  • Female Fertility Assessment

    Hormonal panel (AMH, FSH, LH, estradiol, prolactin), pelvic ultrasound with antral follicle count, and assessment of uterine cavity and tubes where indicated.

Step by Step

IVF and ICSI: Core Treatment Pathway

Every donor embryo at NGC originates from donors who have passed rigorous multi-stage screening, and is then assessed at the laboratory level. Both donor screening and embryo evaluation comply with Russian federal law on ART and NGC's internal protocols, which exceed standard requirements.

Medical Review and Treatment Planning

Review of diagnostic results, selection of stimulation protocol, and preparation of an individualised treatment plan with the fertility specialist.

Ovarian Stimulation

Controlled stimulation using hormone injections over 10–14 days. Follicle growth is monitored by ultrasound. Minimal and natural protocols are available where indicated.

Egg Retrieval

Transvaginal ultrasound-guided egg collection under short anaesthesia. The procedure takes approximately 15–20 minutes and is performed at NGC's clinic in St. Petersburg.

Sperm Preparation or Surgical Sperm Retrieval

Fresh or frozen sperm is prepared on the day of retrieval. If azoospermia or surgical retrieval is required, TESA, TESE, MESA, or PESA is performed.

Fertilisation: IVF or ICSI

Eggs are fertilised in the embryology laboratory. IVF uses conventional insemination; ICSI involves direct injection of a single sperm into each mature egg — recommended for male factor infertility.

Embryo Culture

Fertilised eggs develop in NGC's time-lapse incubators (EmbryoScope™), allowing continuous monitoring without opening the chamber. Embryo quality is assessed daily.

PGT-A (if indicated)

On day 5–6, trophectoderm biopsy is taken from each blastocyst and analysed in NGC's in-house genetics laboratory. Only chromosomally normal (euploid) embryos are selected for transfer.

Fresh Transfer or Embryo Vitrification

The best quality embryo(s) may be transferred fresh, or all embryos are vitrified for a frozen transfer in a subsequent cycle — particularly following PGT-A testing.

Frozen Embryo Transfer (FET)

The uterine lining is prepared using an estrogen and progesterone protocol. One euploid embryo is transferred under ultrasound guidance in a simple, pain-free procedure.

Pregnancy Test and Follow-up

A blood hCG test is performed 10–12 days after transfer. If positive, the first ultrasound is scheduled at 5–6 weeks. NGC coordinates ongoing follow-up with your local obstetrician.

Note on ICSI and PGT-A: ICSI is recommended when male factor infertility is present or when previous IVF cycles showed poor fertilisation results. PGT-A is recommended for patients over 37, those with a history of repeated IVF failure, recurrent pregnancy loss, or known genetic indications. Both procedures are available as part of standard or guarantee programs at NGC.

Preimplantation genetic testing for aneuploidies (PGT-A) is a method of chromosome analysis performed on embryos before transfer. It identifies chromosomally normal embryos, improving the chance of successful implantation and reducing the risk of miscarriage.

When PGT-A May Be Recommended

  • Advanced maternal age (35+ and especially 38+), where chromosomal abnormalities in embryos are significantly more common
  • Repeated IVF failures — multiple unsuccessful transfers despite good embryo quality
  • Recurrent miscarriage — a pattern of early pregnancy loss, particularly when linked to embryo chromosomal status
  • Severe male factor infertility, which may be associated with higher rates of chromosomal abnormalities in sperm

NGC In-house Genetics Laboratory

All genetic testing at NGC — including karyotyping, expanded carrier screening, and PGT-A — is performed in NGC's own genetics laboratory in St. Petersburg. There is no outsourcing to third-party laboratories.

    Non-anonymous Donation at NGC

      Standard spermogram, extended sperm analysis including DNA fragmentation index, and consultation with an andrologist to interpret results and recommend treatment.

    • ICSI for Male Factor Infertility

      Intracytoplasmic sperm injection allows fertilisation even with severely reduced sperm quality. A single viable sperm is injected into each mature egg under microscopic guidance.

    • TESA — Testicular Sperm Aspiration

      Fine needle aspiration of sperm directly from the testis. Used when sperm is absent in the ejaculate but present in testicular tissue. Minimal procedure, outpatient setting.

    • TESE — Testicular Sperm Extraction

      Surgical extraction of sperm from testicular tissue. Used in cases of non-obstructive azoospermia. Performed under anaesthesia with microscopic assistance where indicated.

    • MESA — Microsurgical Epididymal Sperm Aspiration

      Collection of sperm from the epididymis using microsurgical technique. Used in obstructive azoospermia, including post-vasectomy cases.

    • PESA — Percutaneous Epididymal Sperm Aspiration

      Minimally invasive aspiration of sperm from the epididymis using a fine needle. Alternative to MESA in suitable cases of obstructive azoospermia.

    Sperm Cryopreservation

    Sperm retrieved surgically or produced at the time of the female partner's egg retrieval can be cryopreserved for use in current or future IVF cycles. This is particularly relevant for international patients who cannot guarantee simultaneous attendance at the clinic.

    Donor Sperm Option

    When surgical retrieval is not possible or does not yield viable sperm, donor sperm from NGC's cryobank can be used as an alternative. This option is discussed with both partners at the treatment planning stage.

    NGC's andrology laboratory is equipped for all standard and surgical sperm retrieval procedures. Sperm obtained by TESA, TESE, MESA, or PESA can be used fresh for ICSI on the same day or cryopreserved for a future cycle.

    We had various medical treatments without any success in our country, so we decided our only option left was to have IVF with an egg donor. To be able to choose the donor by ourselves, based on pictures, in a big European database was very important for us. The staff, receptionists, translators, medical professors were first-class.

    I couldn't recommend Next Generation Clinic more! The facilities are state-of-the-art and every member of staff has been polite and professional. The communication (in perfect English, as we unfortunately don't speak Russian) has always been prompt and reliable, thanks to fantastic Alex who has been helpful all along.       What is infertility treatment?
      Infertility treatment encompasses all medical interventions aimed at helping individuals or couples achieve pregnancy when natural conception has not occurred after a defined period of unprotected intercourse. Treatment may include diagnostic evaluation, medication, assisted reproductive technologies (ART) such as IVF and ICSI, surgical procedures, or the use of donor material. The appropriate treatment path depends on the identified cause of infertility.
      When should I see a fertility specialist?
      It is generally recommended to consult a fertility specialist after 12 months of regular unprotected intercourse without pregnancy for women under 35, or after 6 months for women 35 and older. Earlier consultation is advised if there is a known history of irregular cycles, previous pelvic surgery, endometriosis, azoospermia, or recurrent pregnancy loss. An online consultation with an NGC fertility specialist can be arranged without travel.

      What infertility treatments are available at NGC?
      NGC offers a full range of ART methods: IVF with own eggs, IVF with ICSI, IVF with PGT-A, IVM (in vitro maturation), frozen embryo transfer, egg donation, sperm donation, embryo donation, surgical sperm retrieval (TESA, TESE, MESA, PESA), cryopreservation and fertility preservation, and guarantee packages. All procedures are available within a single full-cycle clinic in St. Petersburg.
      What is the difference between IVF and ICSI?
      In standard IVF, eggs and sperm are placed together in a culture dish and fertilisation occurs naturally. In ICSI (intracytoplasmic sperm injection), a single sperm is injected directly into each mature egg under microscopic guidance. ICSI is recommended when male factor infertility is present — including low sperm count, poor motility or morphology, or when surgical sperm retrieval is used. ICSI can also be recommended after previous cycles with unexpectedly low fertilisation rates.
      Can male infertility be treated with IVF or ICSI?
      Yes. ICSI is specifically designed to address male factor infertility by allowing fertilisation even with severely reduced sperm quality. For men with azoospermia (no sperm in the ejaculate), surgical sperm retrieval methods — TESA, TESE, MESA, or PESA — can obtain sperm directly from the testis or epididymis for use in ICSI. In cases where surgical retrieval is not successful, donor sperm is available at NGC.
      What tests are needed before IVF?
      Before IVF at NGC, a standard set of investigations is required for both partners. For women: hormonal panel (AMH, FSH, LH, estradiol, prolactin), pelvic ultrasound with antral follicle count, and infectious disease screening. For men: semen analysis and infectious disease panel. Additional tests — genetic screening, hysteroscopy, sperm DNA fragmentation — may be recommended based on individual history. Many of these tests can be completed at a clinic in your home country and submitted remotely before the online consultation.
      Is IVF with PGT-A available in Russia?
      Yes. PGT-A is available at NGC and is performed in our in-house genetics laboratory in St. Petersburg using next-generation sequencing (NGS) to analyse all 23 chromosome pairs. Russia permits PGT-A for medical indications as defined under federal health legislation. There are no restrictions on access for international patients.
      How much does embryo donation in Russia cost?

      The cost depends on the program selected (frozen, PGT-A, or guaranteed), the number of transfer attempts included, and additional services such as extended storage. Please contact us for a detailed, itemized quote based on your specific situation.

      How long do I need to stay in Russia?

      For a frozen embryo transfer, the typical stay in St. Petersburg is 4–5 days around the transfer itself. Endometrial preparation can be managed partially with your local doctor. Our coordinators will provide a precise timeline after reviewing your protocol.

      When is PGT-A recommended?
      PGT-A is typically recommended for women 35 and older (especially 38+), patients with a history of repeated IVF failure or recurrent miscarriage, patients with known chromosomal abnormalities or a genetic disease risk, cases with severe male factor infertility, and patients who wish to transfer a single embryo with maximum certainty of chromosomal normality. Final indications are confirmed by the fertility specialist at the treatment planning stage.
      Can I use donor eggs, donor sperm, or donor embryos at NGC?
      Yes. NGC offers all three donor program types. Egg donation: 600+ verified donors, adult photos available, non-anonymous option available. Sperm donation: cryobank with screened donors, phenotype matching, non-anonymous option. Embryo donation: 450+ donor embryos, including PGT-A tested embryos, with guarantee program options. All donor programs are fully compliant with Russian federal law on ART.
      How much does infertility treatment in Russia cost?
      The cost depends on the treatment protocol selected, the need for donor material, the inclusion of PGT-A, and additional services.       How long do I need to stay in St. Petersburg?

If stimulation monitoring begins at home and only the retrieval and transfer require travel, the stay may be shorter.
